One to One Wellness Program
Program Description

An innovative "one-to-one" confidential wellness program that has been highly successful at assisting individuals in improving their personal health is being offered to all employees. Services are performed by degreed health professionals The program includes:

Program Concepts
This program emphasizes, learning, active fun, eating healthier, going at your own pace, guided self-help, improving the quality of your health. A wellness counselor assists individuals in setting achievable, measurable goals, based on personal needs and preferences.

Health Survey
This confidential health survey takes several minutes to complete. A personal profile report will be generated, identifying lifestyle behaviors to maintain or improve your health. Even a small change, such as walking every day for 15-30 minutes, can greatly help your health.

Wellness Screening
A Heart Health screen is done which includes measurement of blood pressure, cholesterol, HDL-cholesterol (good cholesterol) and glucose. A waist measurement and body mass index is done to determine risk for heart disease.

Individual fitness assessment.
The Polar Fitness Test uses your gender, age, height, and body weight plus your level of physical activity, your heart rate and your heart rate variability measured at rest. Lasting no more than 5 minutes, the result is comparable to VO2max, (oxygen uptake number). This number is the same indicator of aerobic fitness used by professional trainers and athletes ranging from 20, the fitness level of an unfit sedentary individual, to 95, the fitness level of Olympic athletes.

Wear loose fitting comfortable clothing. Short sleeve shirt is recommended. Blood pressure measurements will be obtained. Skinfold measurements (optional) will be taken to measure percent body fat. The health consultant and participant will discuss and set up a personal health plan with goals. The participant will receive written results with interpretation of all values. Results.

  • Your results will be made immediately available to you and will be interpreted for you by your personal wellness consultant.
  • Follow Up and Support
  • By a mutually agreed upon appointment, periodic follow up consultation,
  • Re-measurement of cholesterol, fitness, blood pressure, percent fat will be done to help measure progress and provide continuing assistance.


You do not need to fast for the Cholesterol/HDL-C and glucose test.
Please do not smoke or drink caffeinated beverages 2 hours before assessment.
